Guess I've determined that it's an Allwave 15, not a 12.

Yes, the serial number corresponds to an AW15. You have 12 tubes on the tuner chassis and there should be a power supply/amplifier unit that has 3 tubes: a 5Z3 and two 2A3 tubes - making the total 15. 

If you look in the "Scott Info Archive" (link at top of page), you can find everything on the AW15 in Set Folders.
