Need more info. Your photo reference is unclear.

You hint at having the Alwave Fifteen model  - 12 tube receiver and needing the 3 tube amp  -  1934-5.

Post a photos of your receiver- front view and rear view. 

Also which style knob do you seek.

Barry - I have three Scott AW-15's,  in different console cabinets. 

Scott used a chrome plated 3 tube amp style on several models over the years, so be alert.

You want the one with two 2A3 output tubes and a 5Z3 rectifier. (see photo attached)

It has a 6 pin Jones socket for the set plug and a round 6 pin speaker plug socket.

Has three top mounted filter caps and a one choke plus the power transformer

Tube sockets have embossed tube numbers numbers.

NOT the earlier version amp with two 45's and an 80 rectifier - not compatible.

NOT the late 1930's version with two 6V6's and a 5V4 rectifier - not compatible.

There is a very rough, over priced AW-15 amp on eBay (324875071991)  to help familiarize yourself. Note - the original chrome plated choke has been replaced. Very bad chrome and any body's guess if it has been further hacked.

Be patient for a decent AW-15 amp.

Meanwhile, clean up the cabinet being care not to damage the speaker cloth. To clean the receiver, vacuum and use a small brush to remove all the grit and dirt you can. Then use liquid Turtle brand liquid chrome polish with small squares of old cotton towel.
